• patient’s history of PAP tests: normal, abnormal, and any treatments
• endometriosis or adenomyosis
• malignancies, ie of the cervix, uterus, endometrium, ovary

• hypertension
• diabetes
• other chronic health conditions
• coagulopathy
• depression/anxiety
• thyroid
• surgeries

Current Medications and Allergies
List these, along with dosages and frequency.
Family History
Ask especially about:
• cancer (breast, uterine, and ovarian)
• diabetes
• hypertension
• chromosomal abnormalities
